Four hard constraints — never violated.

    Separation of duties is enforced in code.

    Creator ≠ approver. Executor ≠ approver. Checked at the model and view layer — not just documented.

  • Constraint 02

    Audit-log records are immutable.

    Audit log models do not expose update() or delete(). No migration may add them. No view may modify audit records.

  • Constraint 03

    Self-approval is rejected at every layer.

    Agent workflows, application code, and user-facing features. No user approves their own work — ever.

  • Constraint 04

    Every action is attributable.

    Descriptive entries with object IDs. Traceability is never squashed out of history.

Read it bottom-up and every constraint on the left is visible in the record: an author drafts a revision, the system refuses to let them approve their own work, a second person approves it, an engineer executes against the approved revision, and the evidence package assembles from that trace rather than being reconstructed afterwards.
